Insurance Information

Our office accepts most PPO dental insurance plans. However, our office does not participate with HMO or DHMO plans.

At your fist visit, your insurance card and ID will copied into your file. It would be a great help to your office if you would be sure to provide full dental insurance information over the telephone prior to your first appointment. This will ensure that your dental insurance benefits are accurately and completely determined prior to your arrival, to make payment and decision-making more convenient for you.

Our office is an "in-network" or "direct" provider for Aetna, Assurant, Cigna, Delta Dental, Humana, Geha,  Guardian, MetLife, United Concordia and United Health Care. Please call or stop by our office with any questions you may have regarding your insurance plan.

Where insurance benefits are not determined on the day of your appointment, our office may ask you to pay your estimated balance for the day, and then promptly refund you any credit balance upon receiving reimbursement from your insurance plan.

If our office does not participate with your insurance plan, you will be able to use your insurance coverage to receive "out-of-network" benefit payments from your insurance plan. On the day of service, our office will provide you with the treatment documentation which you will submit to your insurance company to receive a reimbursement payment directly to you.

Also, where your insurance plan does not provide "out-of-network" benefits, or where you are required to do so separately from our office, we will gladly provide you with complete printouts of all procedures and appropriate documentation (e.g. diagnosis notes, x-rays, treatment plans) required by your insurance plan provider to assist you. Our office staff will gladly answer any questions you may have, and will attempt to assist you even where we are not "in-network" providers under your insurance. Please do not hesitate to ask.

Flexible Spending Plans (HSAs and FSAs)

Many of our patients have flexible spending accounts which typically have a "use it" or "lose it" condition for the end of each year. Our office is experienced in helping our patients to maximize your flexible health spending accounts. Please call or visit the office to inquire about maximizing your FSA or HAS accounts.
